I gave a red card in an FHSAA game – now what?

  1. Make sure you have all of the correct information
    • You need the players full name, number, and exactly what the infraction was
  2. Contact the assignor or another board member IMMEDIATELY following the game!
    • A phone call is preferred, but if you don’t know where to start you can send an email to board@cfwloa.org
  3. Fill out an AT6 form within 24 hours of the event
    • That form can be found here: https://form.jotform.us/92624261573155
    • Detail out the infraction exactly as it happened, including direct quotes – euphemisms or alternative word choices are not permitted
    • Put board@cfwloa.org as an additional email address

What if a player leaves the field injured during an FHSAA game and does not return?

  1. Fill out an AT6-M form within 24 hours of the game
    • That form can be found here: https://form.jotform.com/202385210473145
    • Write everything you know, but do not assume to know anything else (for example, if the player left the field holding her wrist, you cannot assume that the wrist is sprained, broken, etc.; just write that she left holding her wrist)
    • Put board@cfwloa.org as an additional email address
